We ask that our patients: Give 24 hours notice when possible for cancellations; the anwering machine is on 24 hours a day. Call if you have a question or a concern or if you will be late. We will do our best to fit you in but need to know in advance for consideration of other patients. Be on time. All patients are taken within 5 minutes of their scheduled time. Schedule appointments in advance and have the proper number of visits scheduled. If you don't schedule your appointments in advance it will be more difficult to give you the times that you desire. Children are required to remain in the waiting room area while the parent and/or guardian is receiving therapy. New-Patient Procedure: If you would like to set up an appointment with North Boulder Physical Therapy and Sports Rehabilitation, LLC, please remember these important steps: 1. Get a prescription from your physician. 2. If your insurance requires a referral from your primary care physician (PCP), have your PCP’s referral coordinator (nurse) fax the prescription/referral to North Boulder PT to do your physical therapy. 3. If you do not have insurance, a prescription from your doctor is helpful but not required. If you are out of network, please call for alternative options or consider private pay. 4. Please arrive 10 to 15 minutes early for your first visit to fill out the required registration paperwork (unless you have filled out the forms ahead of time by downloading our online forms). Please bring your insurance card and prescription. You will be presented with a written verification of your insurance limitations, exclusion, and required co-pays and deductibles as well as your insurance percentage of responsibility.
